Text Box: Swartz Creek Community Schools Text Box: Registration K-12 CLICK HERE!     Text Box:   Registration for K-12 students can be completed at Swartz Creek Community Schools Administration Building between the hours of 8:00 a.m. - 3:00 p.m., Monday thru Friday.  The Administration Building is located at 8354 Cappy Lane,  Swartz Creek, MI  48473.   Please allow 30 minutes to register your child.  For your convenience registration packets are available online.  You can print and complete a registration packet to lessen the time of the registration process.  Please click on the appropriate registration packet and complete the needed forms prior to registering.    You will need to bring the following:   Registration form Home Language Survey form Records Request form  Original Birth Certificate Student’s Social Security card Official Immunization Report Parent/Guardian’s Drivers’ License or Michigan ID with correct address One proof of residency (Mortgage statement, rent agreement, consumer’s bill, or tax bill) IEP-if student was receiving special education services at previous school Unofficial high school transcript from previous school (grades 10-12) Hearing and vision test- Kindergarten students only Legal Guardianship Papers   Registration packets will not be accepted unless complete documentation is provided at the time of registration.  Please contact D.
