My child ____________  ________________  in ____ (grade) has my permission to go to the Durand Lanes bowling alley with the intramural program.  Transportation will be provided by Swartz Creek Schools.  Students will leave the school at 2:45 pm and return to the school approximately 4:15 pm and must be picked up promptly.
